It is sponsored by the Rehabilitation Engineering Research Center for Wireless Technology, "Wireless RERC", at Georgia Tech University. This 10-minute online accessible survey is on the Internet at http://www.wirelessrerc.gatech.edu/survey/coverletter_p2.html

It's also available as hard copy or can be completed over the phone by
contacting:

Lynne Broderick, 2020 Peachtree Road, NW, Atlanta, GA 30309
Voice: (404) 367-1348; Toll-free: (800) 582-6360
